Instructions for registering online.
  1. The latest version of Adobe Reader is required in order to download the Epiphany registration or volunteer form. [Download] 
  2. This page does not contain the registration forms for Faith Formation. Please call the Faith Formation office at (281) 578-8271 to register or for more information.
  3. Download the parish registration form and/or the volunteer form below.
  4. After filling out the form(s) completely, there are three options for submitting the form(s) back to us  
* You can fill in the form, print it,  and mail it in.
* You can fill in the form, print it,  and return it to the Parish Offices yourself.
* You can save the PDF form by going to File > Save As or click the save button (see image), and then send it to us using the form return below.
