This page only contains the Epiphany member registration form & the volunteer form. If you are looking to register your child in our Faith Formation program, please call our Faith Formation office at (281) 578-8271.**

Registration Instructions:

1. Download the parish registration form and/or the volunteer form below.

2. After filling out the form(s) completely, there are two options for submitting the form(s) back to us:

You can print the form out and mail it in or return it to the office yourself.

or

You can click the "e-mail" button on the form which will open the default email client on your computer (most likely Microsoft Outlook) and send it as an attachment. If you do not use Outlook you can either save the PDF form by going to File > Save As, and then send it as an attachment using your own email client.
